NOTE: You will NOT find a copy of the salmon colored Transcript Request Form, one of the most important forms. This is because the unique color of this form allows us to keep track of it easily. The Transcript Request Form can be obtained outside of the Residence Staff Office or in the Main Office. This form MUST be filled out completely, thoroughly and accurately, attached to all other relevant forms, including the completed application and College Counselor Recommendation Forms (if applicable) and submitted to the Registrar to trigger the process that will result in a completed application packet being mailed out in a timely fashion. MSSM College Counseling Checklist * As you will see, this form is a comprehensive piece designed to help you organize your college search and applications. When you decide to fill this form out, you may want to refer to your Links page for the information you will need. MSSM Teacher Recommendation Form(This form is used for College, Scholarship and Various Other Applications) This form often creates a great deal of confusion. Below are step-by-step instructions describing how these forms should be handled: Meet with your college counselor to discuss the three teachers who would be most appropriate to write you a Letter of Recommendation. Keep in mind that we usually encourage students to ask a teacher in the Sciences, Mathematics and the Humanities, respectively. After you have selected your teachers, you should contact them and ask if they would be willing and able to write you a good letter. After a teacher has agreed to write you a Letter of Recommendation, you should ask if he or she would prefer to receive forms electronically or in hard copy. You should also ask your teacher if he or she will be submitting the same letter to all colleges or if he or she will be tailoring letters to each school. There are many schools of thought on this issue. My advice is not to worry, regardless of the answer you receive. If your teacher has asked for forms from each college, then you need to give that teacher one copy of the Common Application Teacher Recommendation Form (with the student's portion already completed) for each school and/or each school's unique Teacher Recommendation Form AS WELL AS one copy of the MSSM Teacher Recommendation form. Your teacher will then submit those forms and letters to the office in a timely manner in order to accompany the remainder of your completed application. Remember to send one copy of each Teacher Recommendation Form to both the Registrar and College Counselor so they know you have submitted your forms and will help you keep track of your progress. The Brag Sheet *this sheet will provide useful information and adjectives for your College Counselor to use when he or she sits down to write your Letter of Recommendation. Parental Input FormThis form, which may be submitted either electronically or by mail, is tremendously useful to your College Counselor.
