Welcome Parents...This area of our site has been created especially for you. We hope you will find it convenient and easy to use. There are a number of forms that will provide much needed information that will assist us in our preparations for the new school year or that will fulfill state requirements.

This year we have placed all of the forms that need to be completed here on the website along with some explanatory information. Please download the Required Forms Checklist that has been designed to help you keep track of what is required. Navigate the Summer Mailing web pages listed above to begin.

☞ Please Note: All forms will need to be downloaded, completed and returned to MSSM by July 18th, 2009.

There are two exceptions to this deadline:

• the Nurse has requested that you complete and return page one (1) of the 11-page 2009-10 Health History document by July 1st, 2009

• NEW students, Mrs. Bowker has asked that you return your Course Registration form by July 10th, 2009.
