REQUIRED FORMS

Dear Parents/Guardians:

As promised in a prior communication, the MSSM forms are now on our website for your completion. Please have your forms completed and returned to MSSM by July 1, 2010.  All forms should be sent to the attention of Diana Nadeau.  All forms must be completed. Even if your son/daughter does not plan to participate in varsity sports, the MPA Transfer form must be completed.

You will also find a letter from Dr. Brown from the IT department.  It is very important to carefully read this letter.

Returning students and parents will notice that we have reduced the number of required forms; we hope this will make the task of completing them somewhat less daunting.  Please keep in mind that we will no longer be providing a list of missing forms on move-in day and that you will not be allowed to register on move-in day until your forms have been completed.  Completing the forms early will make the move-in experience easier for you so please adhere to the July 1 deadline.

Also note that all students are required to have medical insurance.  If you don’t currently have medical insurance, there are several companies which offer low cost student insurance to cover students while they are at school.  Students will not be allowed to enroll at MSSM without medical insurance. We also recommend tuition insurance.  Again, there are several companies which offer such insurance at reasonable fees.

Returning students and parents should note that we no longer require a physical exam each year (unless participating in MPA sanctioned sports teams).  All new students are required to have a physical exam.

There will be another form to be completed later (Free/Reduced Lunch) after the federal government has released the latest edition of the form.

You will be receiving the Health forms in the mail as we believe it will be much easier for you to complete and return this way.  The Health forms should be completed and mailed to the Health Center, attention Sarah Ellsworth, by July 1, 2010.
