Dear MSSM Students and Parents,

It is, once again, that time of year when we need to begin thinking about the next academic year and the necessary information MSSM needs for students to make a smooth transition to school.

We thank you for taking the time to fill out these forms. Many of our forms are required by law while others are provided to help us maintain accountability and to ensure the safety and well-being of our students.

During the school year, if or when any of your personal information changes or it is necessary to change a permission form, please contact my office at 207.325.3303 X 1013 or by eMail at MSSMResLife@gmail.com to ensure changes are made promptly.

Throughout the year your child may wish to travel home with a friend, or may be invited to dinner by the parents of a roommate or close friend. When this occurs MSSM requires that a Written Permission Form be completed by all parities involved. At times these invitations are made with minimal notice. For your convenience, our Permission to Ride forms along with Weekend Permission forms have been placed on the web site.

Please note that Permission to Ride forms are not necessary when students are using public transportation such as a taxi, bus or an airplane.

When possible, please plan ahead. Weekend permission forms with all required signatures are due in our office, Thursday evening prior to the weekend the student wishes to be away. If you are planning to visit and anticipate taking your child's roommate or friend out to dinner or off campus for the day, please let us know in time for us to acquire the necessary permissions. As mentioned above, our policies are in place to help ensure the safety and well-being of our students.
